Privacy Statement

 

Inala Community House (ICH) may collect your personal information in order to provide appropriate and relevant services. ICH recognises the ethical and legal obligations which arise from handling this information. ICH will seek to protect the privacy of individuals in accordance with the Privacy Act 1988 and the Queensland Information Privacy Act 2009.

How do we collect information?

Information may be provided directly by clients through forms, conversations, in writing or other forms of collection. In some cases, this may be provided by a third party. Where this occurs, ICH will take steps to ensure that the client is aware that this information is held by the organisation, how it will be used, and ICH’s contact information. Information collected will vary dependent upon the Service engaged with this may include, but is not limited to:

  • Name
  • Address
  • Contact details
  • Date of birth
  • Occupation
  • Health or disability information (including medical conditions, allergies)

Information may also be supplied by third parties such as:

  • Your support networks
  • Referring agencies/services
  • Other agencies/services you are involved with
  • Emergency contacts
  • Other services you are involved with
  • Reasons for engaging our services
  • Your support networks (e.g. family, friends, carer, community, etc.)

Why we collect your information?

ICH has a number of services which require information about you, in order to provide you with appropriate, tailored services and on some occasions to ensure your wellbeing. Other information may be collected as required to comply with legal or funding agreement requirements.

You are not required to give us all your personal information however this may mean that ICH cannot provide or can offer only limited services.

When do we disclose your information?

ICH will not disclose your information to other parties without consent unless it is permitted by law, regulation, other statutory instruments or service agreement. ICH may disclose information to relevant agencies or authorities where there is a threat to life or health.

ICH may also disclose your information to contractors who provide services on behalf of ICH, these contractors shall be bound by confidentiality and privacy obligations.

ICH may seek your consent to disclose your information to third parties including other service providers, this is to assist in ensuring services are integrated and to ensure continuity of services.

How is your information kept safe?

ICH will take reasonable steps to safeguard your personal information from misuse, loss, unauthorised access, modification or disclosure. This includes:

  • All files, both electronic and paper, are stored in Australia
  • For paper files, our offices are secured and information is kept in locked cabinets
  • Electronic files can only be accessed by using a password to login to the computer/database
  • Private areas for confidential discussions
  • Requiring staff to maintain confidentiality

Can you access your information?

ICH will provide access to information in accordance with the Privacy Act 1988, and other related laws, regulations, other statutory instruments or service agreements. If you would like to access your information, please talk to a staff member or if you are unsure who to talk to, talk to one of our receptionists and they can direct you to the appropriate person.

Can you correct or update your information?

ICH will correct or update your personal information if necessary, in accordance with the Privacy Act 1988, and other related laws, regulations, other statutory instruments or service agreements. If you need to update or correct your information, please talk to a staff member or if you are unsure who to talk to, talk to one of our receptionists and they can direct you to the appropriate person.
